City Select (“City Select,” “we,” “us,” or “our”) operates an online directory of medical and dental practices in Arizona at cityselectmedical.com (the “Site”), together with a related featured-placement print mailer program. The Site is operated by City Select Media, LLC. By accessing or using the Site you agree to these Terms of Service. If you do not agree, please do not use the Site.

The Site provides general information about healthcare practices for the public’s convenience. It is not a substitute for professional medical advice, diagnosis, or treatment, and nothing on the Site constitutes a referral, endorsement, or recommendation of any practice or provider. Always seek the advice of a qualified health provider with any questions about a medical condition. A listing on the Site does not certify a provider’s licensure, qualifications, or the quality of care they provide.

Practice listings are compiled primarily from the federal National Provider Identifier (NPI) Registry maintained by the Centers for Medicare & Medicaid Services and from other publicly available sources, supplemented by information practices provide when they claim a listing. We work to keep information accurate but we do not warrant that any listing is complete, current, or error-free. Practice names, addresses, phone numbers, hours, services, accepted insurance, and provider rosters may change without notice. Verify details directly with the practice before relying on them.

Practices may claim their listing free of charge to correct or enhance their information. To request correction or removal of a listing, contact us at hello@cityselectmedical.com. We honor reasonable removal requests for listings sourced from public records, though factual information drawn from public registries may remain available from their original sources.

Some practices pay for featured placement on the Site or in the City Select print mailer. Paid placements are offered on a category-exclusive basis as described in the applicable order or agreement. Pricing, availability, and program terms are set out at the time of purchase and may change. Paid placement affects display prominence only and does not change the factual information shown about any practice.

You agree not to: scrape, harvest, or bulk-download Site content except as expressly permitted; use the Site to send unsolicited communications to listed practices; misrepresent your identity or authority to act for a practice; interfere with the Site’s operation or security; or use the Site for any unlawful purpose. We may suspend or restrict access for conduct that violates these Terms.

The Site’s design, original text, graphics, logos, and the selection and arrangement of content are owned by City Select Media, LLC and protected by applicable law. Factual data drawn from public registries is not claimed as proprietary. You may not copy, reproduce, or create derivative works from the Site’s proprietary elements without our permission.

The Site may link to practice websites and other third-party resources. We do not control and are not responsible for the content, accuracy, or practices of any third-party site. Links are provided for convenience and do not imply endorsement.

The Site is provided “as is” and “as available” without warranties of any kind, express or implied, including warranties of accuracy, merchantability, or fitness for a particular purpose. To the fullest extent permitted by law, City Select Media, LLC and its affiliates will not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of, or inability to use, the Site or any listing.

You agree to indemnify and hold harmless City Select Media, LLC from any claims, damages, or expenses arising out of your misuse of the Site or violation of these Terms.

We may update these Terms from time to time. Material changes will be reflected by the “last updated” date above. Your continued use of the Site after changes take effect constitutes acceptance of the revised Terms.

These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-laws rules. Any dispute arising from these Terms or the Site will be resolved in the state or federal courts located in Maricopa County, Arizona.
